Getting started in three steps
Book your first visit
Reserve online or call your nearest office. New-patient registration is completed when you book.
Check your insurance
Confirm we’re in-network with your plan before you come in — or contact us to verify coverage.
Bring what you need
Photo ID, insurance card, your child’s immunization record, and any prior medical records.
Before your visit
Reserve your first appointment, confirm your coverage, and request records from your previous doctor.

Coming from another pediatrician or provider? Sign this form to authorize the transfer of your child’s records to us.
Enter your previous doctor’s name, address, and fax number exactly as they have them — we fax the signed request there automatically, and it won’t go through without a correct fax number.

Our approach to vaccines
One World Pediatrics is an NCQA-recognized Patient-Centered Medical Home, and we welcome every family — wherever you are in your vaccination decisions. We recommend the CDC and American Academy of Pediatrics immunization schedule for every child, and your provider will talk through vaccines with you at each visit, but your vaccination choices will never keep your child from being seen here.
- Following the recommended schedule, spacing vaccines out, or choosing not to vaccinate — we’ll see your family either way. Vaccination status never blocks scheduling.
- Your provider will discuss vaccine safety and answer your questions at relevant visits, and will note your family’s preferences in your child’s chart so everyone stays on the same page.
- Already vaccinating on a different schedule, or not vaccinating at all? Bring your child’s immunization record, if you have one, to your first visit.
